St. Patrick’s Manor is a nonprofit, short-term and long-term care facility sponsored by the Carmelite Sisters. Hourly Pay: $27 per hour plus shift and weekend differentials for LPN’s with two years of experience Job Responsibilities:
Integrate values of Carmelite Philosophy of Care in all job assignments
Respond to all staff, family members and residents in a courteous, patient and caring manner
Respect the dignity of each person
Communicate in a positive and helpful manner
Knowledgeably and completely perform basic nursing skills
Administer and document medications and treatments per the physician’s order and accurately record all care provided
Order medications as necessary
Review and revise resident care plans as necessary and as required by regulations
Conduct resident rounds daily, reporting problems to charge nurse
Obtain report from nurse he/she is relieving and record sufficient information to implement appropriate follow-up action as necessary
Provide report to nurse coming on duty, including sufficient information for follow-up action if necessary
Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Charge Nurse
Observe, evaluate and document all changes in the residents’ conditions
Notify physician, when necessary, of changes in the residents’ condition, weight, medication errors, medications not available, abnormal laboratory/X-ray results and other unusual observations
Observe infection control procedures
Identify safety hazards, initiate immediate “life saving” measures
Provide clinical supervision of nursing assistants providing direct resident care
Perform other related duties as directed
